It is a rectangular block of wood with a carved relief of the number "4" (movable typeface). The relief surface is glossy/smooth as the original coat of varnish is still evident and is orange with dark scratches and areas of dark red. The carved down portion is a matte dark red. The back is covered in black, red and brown pigment that has naturally darkened with age and use. There is a large patch of glued on paper that has been partially ripped off. The old accession number is written in large letters in permanent marker.
